Researchers at Indiana University School of Medicine have identified an enzyme called IDOL as a potential new target for treating Alzheimer's disease.

Laboratory experiments demonstrated that removing IDOL from neurons significantly reduced amyloid plaques, a key hallmark of the disease, and improved brain processes associated with neuron communication and resilience.Surprisingly, while microglia—brain immune cells—produce IDOL, removing it from neurons had the most pronounced effect.

The intervention also reduced levels of apolipoprotein E (APOE), particularly APOE4, a major genetic risk factor for late-onset Alzheimer's, and increased receptors involved in regulating amyloid plaques and lipid metabolism.

These findings suggest that targeting neuronal IDOL could simultaneously decrease plaque burden and enhance neuroprotective mechanisms, offering a therapeutic strategy that goes beyond current treatments like lecanemab and donanemab, which primarily slow disease progression.

Future research will focus on developing IDOL-targeting compounds, evaluating their safety, and exploring whether blocking IDOL can preserve synaptic connections and reduce tau pathology, another characteristic of Alzheimer's disease.

This discovery represents a promising step toward treatments that may protect the brain and improve patient outcomes even after substantial plaque accumulation has occurred.
