Adv Andrea Johnson, head of the Investigating Directorate Against Corruption (IDAC), testified at the Madlanga Commission that her unit lacked grounds to investigate Lieutenant General Dumisani Khumalo over alleged police recruitment irregularities.

She admitted the anti-corruption unit was not supposed to solicit complaints from the public, intensifying scrutiny over the investigation's origins.

The probe was initially triggered by a letter from Police Minister Senzo Mchunu's office, but it arrived without supporting documents, rendering it invalid for formal action.Johnson struggled to explain her response to the missing attachments, claiming technical teams might still recover the email.

The commission focused on how a Section 27 affidavit from National Coloured Congress leader Fadiel Adams became the basis for IDAC's investigation.Johnson suggested her chief investigator may have contacted Adams, but this was contested.Chair Justice Madlanga emphasized that Johnson, as the person who instructed the investigator, must account for the process.Johnson eventually conceded that IDAC should not have solicited the affidavit, which was stamped only 10 days after the initial correspondence.The hearing was interrupted when Johnson became unwell, with further questions expected on the investigation's origins.
